Moora Aged Care Support is a not-for-profit incorporated community organisation that provides regional and remote community home care services within the Moora Shire.
We work with people who have an approved Support at Home funding under the Federal Government’s Support at Home program.
We aim to employ locals and use local businesses to provide the support for our community whether they are living in town or on one of the many surrounding rural properties.
As care coordinators, we have partnered with Trilogy Care as the Support at Home provider. Moora Aged Care is built by a team of locals dedicated in providing easily-accessible home care supports.

We will assist you in finding care workers and support workers that help you live safe, independent and connected to the community.
SAH MANAGEMENT
Have your support team managed by us, as well as any incidents or requests. Stay in control and remain independent while receiving support services.

Moora Aged Care will manage all aspects of your Support at Home funding that many find difficult, such as finding appropriate support workers, scheduling care hours, and managing your budget.
We work with you and the local workforce to meet your care needs. Once you have selected your preferred support worker, we run compliance checks to ensure all workers have a registered ABN, current insurance, and any licenses required to carry out their specific line of work.
Working with your local care coordinator that is powered by Trilogy Care (a registered aged care provider) means you can be assured you are receiving the best quality care, offering you more flexibility and value for money than with a traditional aged care service provider.
Providing regional and remote community care.
In-home care is hard to come by in our region. There is a lack of choice in providers and care services who do not live within the community and charge significant costs for travel, and to service our communities.
These high charges result in less visits from care workers, and inadequate support to properly care for our seniors in Moora. In many cases, our seniors have lived their entire lives on their property or within our town and want to remain living the way they know how.

Where do you start with aged care?
All aged care services in Australia are coordinated through My Aged Care, your starting point for accessing Australian Government funded aged care programs and services.
Am I eligible for Support at Home funding?
To satisfy the requirements of Support at Home funding, you must be 'an older person who needs coordinated services to help them stay at home'. The eligibility requirements state you must be over 65 years of age or 50 years of age for an Aboriginal or Torres Straight Islander heritage.

What is an ACAT assessment?
The Aged Care Assessment Team (ACAT) will assess your health, lifestyle and living situation, from this information they grant packages to consumers on eight different levels of care:

What services are available with a Home Care Package?
There are various services you are able to receive through Support at Home (SAH). SAH funds can be used for home care services which assist you in remaining healthy, safe, independent, and connected to the community. Services that are specifically excluded by the Australian Government from being covered with SAH funds are excluded because they do not meet the immediate care needs of consumers.
